Jawline & Neck Contouring: Platysmaplasty Explained

Why Does the Jaw-Neck Angle Matter So Much?

The jaw-neck angle — the acute angle formed where the chin meets the neck — is one of the most telling markers of a youthful profile. In women, an ideal angle sits around 105–120°; in men it tends to be slightly sharper. When this angle becomes obtuse or disappears entirely, the face reads as tired, heavy, or aged, regardless of the person's actual weight or age. The root cause is almost always the platysma muscle.

What Is the Platysma and Why Does It Sag?

The platysma is a thin, broad muscle that drapes the entire front of the neck. In youth it forms a continuous supportive sheet beneath the chin. Over time the muscle separates in the midline, creating vertical cords known as platysma bands. Submental fat accumulation and loss of skin elasticity accelerate the process. The visible results include:

  • **Double chin:** Fat and skin excess beneath the mandible
  • **Platysma bands:** Prominent vertical cords on the front of the neck
  • **Turkey neck:** Skin laxity combined with platysma separation
  • **Blurred jawline:** Loss of clear definition between face and neck

Platysmaplasty: The Core of the Procedure

Platysmaplasty is the surgical reshaping of the platysma muscle. It forms the structural heart of a neck lift and goes considerably further than simply tightening skin.

Surgical steps: A small incision beneath the chin (placed in a natural crease) with optional additional incisions behind the ears; midline plication of the platysma; transection or weakening of prominent bands; submental liposuction to remove excess fat; skin flap re-draping and removal of redundant skin.

When Is Liposuction Added?

Where submental fat is prominent, adding liposuction to platysmaplasty directly improves contour. For patients between roughly 35–50 with reasonable skin elasticity, liposuction combined with platysmaplasty can deliver a compelling improvement without the full incision length of a traditional neck lift. In cases of advanced skin laxity, however, liposuction alone is insufficient; excess skin must be surgically removed.

Combination with Deep Plane Face Lift

When the platysma sags, the lower face — jowls, mandibular border, nasolabial region — is almost always involved as well. Combining a face lift with neck contouring in a single session therefore makes both technical and aesthetic sense. The deep plane technique works beneath the SMAS layer, repositioning the true structural supports of the face, and integrates seamlessly with platysmaplasty on the neck side. This combination maintains continuity from jowl to neck and produces a natural, rested result rather than a pulled look.

Who Is a Suitable Candidate?

  • A persistent double chin despite weight management
  • Loss of the jaw-neck angle visible in profile view
  • Visible platysma bands — especially noticeable when speaking or swallowing
  • Loose, hanging skin on the neck
  • General health suitable for surgery

Recovery Timeline

The first two days involve a neck bandage and drain monitoring. Mild swelling and bruising in the first week are normal. Most patients return to social activities within 7–10 days. Because the platysma sutures are placed deep, long-term improvement is maintained — results are more durable than surface-only skin-tightening approaches.
