The middle ear is ventilated through the Eustachian tube. If this ventilation is impaired due to adenoid enlargement etc., fluid accumulates in the middle ear (Serous Otitis). This causes hearing loss.

In cases unresponsive to medication, a "Ventilation Tube" is inserted into the eardrum. This microscopic tube allows the middle ear to breathe.
